MODEL RELEASE FORM

This Model Release Form ("Agreement") is entered into between [Photographer Name], located at [Photographer Address] ("Photographer") and [Model Name], located at [Model Address] ("Model").

1. Grant of Rights. Model hereby grants Photographer and Photographer's assigns, licensees, and legal representatives the irrevocable right to use Model's name, likeness, image, and appearance (collectively, "Image") in all forms and media, including but not limited to photographs, videos, and digital images ("Media"), for all purposes, including advertising, marketing, promotion, and trade, in perpetuity.

2. Compensation. Model acknowledges that Model will receive no compensation for the use of the Image.

3. Ownership and Copyright. Photographer shall own all rights, title, and interest, including copyright, in and to the Media and any derivative works created therefrom.

4. Representations and Warranties. Model represents and warrants that (a) Model is over 18 years of age, or Model's parent or guardian has signed this Agreement on Model's behalf, (b) Model has the full right and authority to enter into this Agreement and grant the rights herein, (c) Model has not entered into any other agreement that conflicts with the terms of this Agreement, and (d) the use of the Image will not violate the rights of any third party.

5. Indemnification. Model agrees to indemnify and hold Photographer and Photographer's assigns, licensees, and legal representatives harmless from any claims, damages, or expenses arising from Model's breach of this Agreement or from any third party claim related to the use of the Image.

6. Governing Law. This Agreement shall be governed by and construed in accordance with the laws of the State of [State], without giving effect to any principles of conflicts of law.

7. Entire Agreement. This Agreement contains the entire understanding between the parties and supersedes all prior agreements and understandings, whether written or oral, relating to the subject matter of this Agreement.

8. Execution. This Agreement may be executed in counterparts, each of which shall be deemed an original and all of which together shall constitute one and the same instrument.
